Your health care at A.P. Beutel Health Center
is a cooperative effort between you and your health care provider. The
Student Health Services staff would like for you to be aware of your
individual rights as well as your responsibilities.

choosing a form of treatment.
- To receive
education and counseling.
- To consent to, or
refuse, care and/or treatment and be informed of medical
consequences.

change your health care provider.

medical records with a clinician.
